The Provider Group Classes are led by a Health Care Provider specialized in diabetes care and a Health Educator. Within the group visit, you will have an appointment with the health provider and also learn how to stay healthy. The group visit focus on the basics of diabetes; nutrition; exercise; how to prevent complications; how to set realistic goals and how to stay healthy. There will be healthy snacks at each visit and prizes for those who attend.
The class is open to patients and the community who have diabetes, pre-diabetes or anyone who want to learn on how to prevent the disease.
