Meet Patrick, Rosa and Edgar.
Rosa feared for her three children’s future as she was wracked by debilitating stomach pain, all while living in a shelter. Our Enhanced Care Management Team got Rosa specialist gastroenterologist care, while also connecting her with our Early Head Start child development program.
Patrick urgently needed heart surgery. But without a home, he would have no place to recover. His primary care provider moved mountains to schedule a life-saving surgery while team members also got him health insurance coverage, helped him navigate disability and secured housing for his recuperation.
Like his relatives before him, Edgar’s liver cirrhosis from alcohol use disorder threatened his life — and was causing him to entirely miss out on his children’s lives. He connected on a soul-to-soul level with one of our addiction counselors. Now sober, his liver is back to normal and he is finally the father he has always wanted to be.
For all three of these individuals, meeting dedicated Venice Family Clinic clinicians and staff changed everything.
Now, they have their whole lives ahead of them.
