Venice Family Clinic is now using a new portal system – MyChart.

If you currently have an email on file, you will automatically be enrolled in MyChart at your next visit. You just have to activate your account by following these steps:

To sign up with an activation code:

1. Open the enrollment invitation email
2. Click “Sign Up” or copy the URL in the email and paste it in your web browser
3. Enter your zip code and date of birth
4. Click “CONTINUE”
5. Create your username and password
6. Click “ENTER PORTAL”
