How to self-schedule your COVID-19 vaccine appointment

You can make an appointment in the patient portal for the COVID-19 vaccine. Follow the steps below to choose a day and time that works for you:

STEP 1:

Log into your portal account at www.nextmd.com

Select SCHEDULE > REQUEST APPOINTMENT from menu in the upper right-hand corner

STEP 2:

Select COVID VACCINE from the provider/group drop down menu

STEP 3:

Select your preferred vaccine from the CATEGORY menu

STEP 4:

Select your preferred location from the LOCATION menu

STEP 5:

Indicate your eligibility category in the REASON FOR VISIT field

Enter the dates and times you’d like to search

Select SEARCH

STEP 6:

Choose your preferred appointment

Click BOOK APPOINTMENT
